Virginia Epi-1 Confidential Morbidity Report

Virginia physicians and medical facility directors must report conditions on the Virginia Reportable Disease List to their local health department under Code of Virginia 32.1-36 and 32.1-37 and 12VAC5-90-80. The Epi-1 Confidential Morbidity Report is the form for that report, available as an online portal and a paper form. The local health department uses it to investigate the case.

Who completes it

The physician or other provider who diagnoses or suspects the condition completes everything except the 'For Health Department Use' box, including the name of the person completing the form and the date reported.

Submitting and payment

How to submit it
VDH prefers the online Confidential Morbidity Report Portal; electronic case reporting and the paper Epi-1 mailed to the local health department (the top two copies) are also accepted. Rapidly reportable conditions are phoned to the local health department, and unusual or emerging conditions immediately, preferably by phone.

Good to know

  • VDH is moving from its current REDCap portal to CMRP 2.0: onboarding starts November 2026, it opens to all users by January 5, 2027, and the old portal retires in February 2027.
  • Influenza is reported as a number of cases and type only, with no patient identification, in its own box on the Epi-1.
  • The comments field asks for risk situations such as food handling, patient care or day care, treatment and immunization dates, symptoms and outbreak association.
  • STD, HIV and hepatitis reports can also go through the Epi-1 portal, or to the VDH Central Registry Unit.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can I still use the paper Epi-1 in Virginia?

Yes. VDH lists the paper Confidential Morbidity Report Form (Epi-1) as an option alongside the online portal, a computer-generated printout or a CDC or VDH surveillance form. The paper form says to mail the top two copies to the local health department, though the online portal is the preferred route.

What is CMRP 2.0 in Virginia?

It replaces VDH's current REDCap Confidential Morbidity Report Portal. Higher-volume reporters and Neonatal Abstinence Syndrome reporters get onboarding invitations starting November 2026, CMRP 2.0 opens to all users by January 5, 2027, and the current portal retires in February 2027.
