Connecticut DCF-136 Report of Suspected Child Abuse

Connecticut mandated reporters, including physicians, nurses and physician assistants, must make an oral report to the DCF Careline or law enforcement within 12 hours of suspecting abuse or neglect. Within 48 hours of that oral report, they submit DCF-136. The 2-page form records the child, caregivers, suspected perpetrator, the injury or neglect and how it became known, prior history, and action taken to help the child.

Who completes it

The mandated reporter completes the whole form, including the reporter section, signature and position.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
