North Dakota SFN 960: Child Abuse or Neglect Report

North Dakota mandated reporters, including physicians, nurses and other medical professionals, can report suspected child abuse or neglect verbally or in writing; SFN 960 is the written version. The one-page fillable form (9-2025) goes to Child Protection Services under NDCC 50-25.1, which assesses the report and decides whether services are needed to protect and treat the child.

Who completes it

The reporter completes the child, parent or caretaker, subject and narrative sections and signs; the Agency Use Only block at the bottom is for CPS intake. Entities with more than 25 mandated reporters may name a designated agent to file for staff.

Submitting and payment

How to submit it
Fax the form to the CPS Central Intake Unit or submit it to the human service zone office where the child is physically located. Verbal reports go to the statewide Child Abuse & Neglect Reporting Line, weekdays 8 a.m. to 5 p.m. Central Time.
Who pays
ND HHS lists no fee for reporting.

Good to know

  • The narrative asks for the nature and extent of suspected abuse or neglect, prior incidents and family composition, and to be specific about who, what, when, why and how often.
  • Calls to local zone offices about suspected abuse are rerouted to the central reporting line, where intake staff gather facts for local CPS workers.
  • A designated agent's single report satisfies the duty for the staff listed in it, but the agent cannot require approval or change what staff report.
  • SFN 960 can also be used for institutional reports, which are forwarded to the Institutional Child Protection Services unit. Call 911 if a child is in immediate danger.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there an online child abuse report in North Dakota?

ND HHS lists no online form for child abuse. Reports go verbally to the statewide reporting line, staffed weekdays from 8 a.m. to 5 p.m. Central Time, or in writing on SFN 960, faxed to the CPS Central Intake Unit or delivered to the local human service zone office.

Can a hospital file one report for its staff?

Yes. An entity employing more than 25 mandated reporters may appoint a designated agent. The agent's report must list the name, title and contact information of each staff member or volunteer with direct knowledge, and that single report meets their requirement. Individuals may still report directly.
