PA CY 47: Report of Suspected Child Abuse

Under Pennsylvania's Child Protective Services Law (23 Pa.C.S. Chapter 63), a mandated reporter who makes an oral report of suspected child abuse to ChildLine must follow it with a written report. The CY 47 is that written report, sent to the Department of Human Services or the county children and youth agency assigned to the case. The Child Welfare Portal offers an electronic alternative.

Who completes it

The mandated reporter, such as a physician, nurse or other health practitioner, completes the whole form: items 1 to 8 and the reporting source block with printed name and signature. No section is completed by the family.

Submitting and payment

How to submit it
Call ChildLine immediately, then send the CY 47 within 48 hours to DHS or the county agency assigned to the case; it may be submitted electronically. A report made through the Child Welfare Portal replaces both the call and the written form.

Good to know

  • The form asks about the child, both parents, any other person responsible, the alleged perpetrator and the perpetrator's employer, plus household members, the incident address and date.
  • A body diagram lets the reporter mark the exact location of injuries, and item 7 records actions taken, such as X-rays, photographs, hospitalization, police notification or protective custody.
  • Item 8 asks about safety concerns and risk factors: the child's functioning, caregivers' mental health, substance use, domestic violence, discipline methods, home conditions, weapons and caseworker safety.
  • If the child has been taken into protective custody, the form says to contact the county children and youth agency where the abuse occurred immediately.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is the Pennsylvania CY 47 due?

Within 48 hours of the oral report. A mandated reporter who calls ChildLine must also make a written report to the Department of Human Services or the county agency assigned to the case, and the form's instructions say the CY 47 serves that purpose and may be submitted electronically.

Do I still need the CY 47 if I report online?

No. Pennsylvania DHS says mandated reporters can make the report electronically through the Child Welfare Portal, and an electronic report there takes the place of both the ChildLine call and the written CY 47. Most mandated reporters now use the portal.

What if I don't know some answers on the CY 47?

Item 8 offers an 'information unknown' checkbox for each of its safety and risk-factor questions, so a reporter is not expected to know the household's full history. The instructions also allow additional sheets of paper when more space is needed to give the requested information.
