Iowa 470-0665: Report of Suspected Child Abuse

Form 470-0665 is the written child abuse report Iowa provides for mandatory reporters to send after their oral report to the Centralized Services Intake Unit (CSIU). A clinician meets it after calling in suspected child abuse, which must happen within 24 hours. The form records the child and family, the suspected abuse with the CSIU incident number, and the reporter's details and signature.

Who completes it

The mandatory reporter completes and signs the whole form, listing other mandatory reporters who know about the abuse and anyone already told, such as a supervisor or building administrator.

Submitting and payment

How to submit it
Make the oral report within 24 hours on the Iowa HHS abuse line, then, if sending a written report, submit the form to CSIU by email, fax or mail; the form text says within 48 hours of the oral report.
Who pays
Iowa HHS lists no fee or payment for child abuse reports.

Good to know

  • Iowa HHS's mandatory reporter page now says a written report is no longer required, even though the 06/21 form text still describes it as a legal requirement.
  • Enter the incident number CSIU gives you during the oral report, so the written report matches the right intake.
  • Iowa Code 232.73 protects good-faith reporters, including when taking photographs or X-rays or performing medically relevant tests.
  • Mandatory reporters complete Iowa's child abuse training within six months of starting the job and every three years after; it is free through Workday Learning.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is form 470-0665 available in Spanish?

Yes. Iowa HHS lists a Spanish version, 470-0665(S), Reporte de sospecha de abuso de menores, alongside the English form on its mandatory reporters page. Iowa's mandatory reporter training is also offered in Spanish, as course DS 169 for child abuse.

What happens after an Iowa child abuse report?

Iowa HHS must tell the reporter orally within 24 hours whether the report was accepted or rejected, then send a written Notice of Intake Decision. An outcome notice later classifies the report as Founded, Confirmed or Not Confirmed, and reporters may request a copy of a founded assessment they reported.

Can I use my clinic's own report letter instead of 470-0665?

Yes. The form states that an agency report form or letter format containing all the information requested on 470-0665 may be used in its place. Send it to CSIU the same way, by email, fax or mail, and include the incident number from your oral report.
