Iowa 470-2441: Suspected Dependent Adult Abuse Report

Iowa HHS form 470-2441 is the written format for reporting suspected dependent adult abuse to the Centralized Services Intake Unit (CSIU). Mandatory reporters must report within 24 hours of reasonably believing a dependent adult has been abused, usually by phone, and this form is the written route when no oral report will be made. It documents why the person is a dependent adult, the type of abuse and the caretaker.

Who completes it

The mandatory reporter completes the whole form, explains why the person is a dependent adult and why the named caretaker is a caretaker, lists other mandatory reporters who know of the abuse, and signs it. Intake staff supply the report number.

Submitting and payment

How to submit it
Send the form to the Centralized Services Intake Unit by email, fax or mail within 24 hours if no oral report will be made; Iowa HHS also takes oral reports on its 24-hour line. Call 911 or law enforcement if the adult needs immediate protection.
Who pays
Iowa HHS lists no fee or payment for abuse reports.

Good to know

  • Check the abuse type: exploitation, physical assault, sexual offense, personal degradation, unreasonable punishment or confinement, or denial of critical care by the caretaker or by the dependent adult.
  • Skip the caretaker section when the deprivation is by the dependent adult, as the form instructs.
  • An agency report form or letter that contains all the same information may be used instead of form 470-2441.
  • Mandatory reporters must complete Iowa's dependent adult abuse training within six months of starting the job and every three years after.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is a written report still required for dependent adult abuse in Iowa?

Iowa HHS now states that a written report of suspected abuse is no longer required when an oral report is made within 24 hours. Form 470-2441 remains available for reporters who report in writing instead, and it must be submitted within 24 hours if no oral report will be made.

Will I hear back after reporting dependent adult abuse in Iowa?

Yes. Iowa HHS must tell the reporter orally within 24 hours whether the report was accepted or rejected, followed by a written Notice of Intake Decision. For dependent adult abuse, an outcome notice later reports the assessment results. Mandatory reporters must give their name and address when reporting.

Are Iowa dependent adult abuse reporters protected from liability?

Iowa Code 235B.3(11) gives dependent adult abuse reporters immunity for making a report or assisting, and for good-faith participation in related judicial proceedings. Employers and supervisors may not apply policies or work rules that interfere with a mandatory reporter making a report.
