PA-NEDSS Disease Reporting for PA Providers

Pennsylvania's Department of Health requires health care practitioners and facilities to report diseases and conditions listed under 28 Pa. Code Chapter 27 electronically, through PA-NEDSS, its mandatory disease reporting application. The system replaced the card and form-based methods clinicians once used, accepts reports around the clock, and lets the Department pass findings on to CDC.

Who completes it

The health care practitioner or facility that diagnoses or suspects the condition enters the whole case report online. Laboratories report results separately through electronic laboratory reporting (PA-ELR).

Submitting and payment

How to submit it
Enter the report in PA-NEDSS, which accepts reports 24 hours a day, 7 days a week. Most conditions are due within 5 work days; conditions flagged for urgent reporting, such as anthrax, botulism, measles, rabies, typhoid and animal bites, within 24 hours; cancer within 180 days.

Good to know

  • There is no paper case report form to download: the Department states PA-NEDSS replaced any card or form-based reporting methods.
  • Sexually transmitted infections are reported through PA-NEDSS as well; the Department's STI program confirms it is the mandatory route for STI case reports.
  • Philadelphia, Allegheny County and other county or municipal health departments may have their own local reporting arrangements, so check the patient's jurisdiction.
  • The Department's list of reportable diseases in Pennsylvania marks which conditions are due within 24 hours, the key detail to check before entering a case.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is PA-NEDSS mandatory for Pennsylvania providers?

Yes. The Department of Health describes PA-NEDSS as the mandatory electronic disease reporting application for Pennsylvania. It replaced card and form-based reporting, so clinicians and facilities enter reportable disease cases online rather than mailing or faxing a paper case report to the state.

How fast must a reportable disease be entered in PA-NEDSS?

Most conditions are due within 5 work days. Conditions marked for urgent reporting on the Pennsylvania reportable disease list, including animal bites, anthrax, botulism, measles, rabies and typhoid, are due within 24 hours. Cancer cases have 180 days.

Are animal bites reported in PA-NEDSS?

Yes. Animal bites are on the Pennsylvania reportable disease list and are due within 24 hours through PA-NEDSS, the same system used for infectious diseases. Patients living in a county or municipality with its own health department may fall under local reporting arrangements as well.
