Ohio BWC MEDCO-16: Mental Health Notes Summary

Mental health professionals treating an Ohio workers' compensation claim can use this one-page summary to share treatment information without releasing psychotherapy notes. It reports the allowed DSM conditions being treated, session length and modalities, symptoms, prognosis, progress and functional status. BWC and managed care organizations use it to manage the medical side of the claim, including whether care should continue.

Who completes it

The treating mental health professional, such as a psychologist, psychiatrist, licensed counselor or social worker, completes and signs the whole form with an NPI. The injured worker does not complete any part.

Submitting and payment

How to submit it
Fax or mail it to the injured worker's MCO; if the worker's employer is self-insuring, send it to the self-insuring employer instead.
Who pays
The MEDCO-16 sets no fee and has no billing section.

Good to know

  • Use of the form is optional: the instructions say a mental health professional may use it to submit mental health notes.
  • The conditions field asks for the BWC allowed condition or conditions being treated, identified by DSM diagnosis, along with the dates of the treatment period.
  • Progress is marked as no change, worsened, improved, approaching complete, complete, or not applicable for an initial visit; prognosis is good, fair or poor.
  • The functional status box is where the provider describes the worker's ability to remain at or return to work, with extra sheets attached if needed.

Mental Health Notes Summary (Non-Psychotherapy Note)

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is MEDCO-16 the same as psychotherapy notes?

No. The title calls it a non-psychotherapy note. It summarizes treatment dates, frequency, session length, modalities, medication monitoring, symptoms, prognosis, progress, goals and functional status, so the MCO can manage the claim without receiving the clinician's psychotherapy notes.

Where do I send MEDCO-16 for a self-insured employer?

For a state-fund claim, fax or mail it to the managed care organization assigned to the claim, which the injured worker or employer can identify. If the injured worker is employed by a self-insuring employer, the instructions say to mail or fax the completed form to that employer.
