Ohio BWC C-143 PC: DEP Psychological Work Ability

Ohio BWC's Disability Evaluator Panel examiners use this form when a claim involves cognitive or psychological conditions. The examiner circles a restriction level for each work-related ability, from Class 1 (no restriction) to Class 5 (restriction levels preclude useful functioning), based on the clinical evaluation and testing. BWC reads it with the examination report to judge how many hours and days the injured worker can work.

Who completes it

The BWC Disability Evaluator Panel physician or psychologist completes and signs the whole form. The injured worker does not complete any part.

Submitting and payment

How to submit it
The examiner returns the signed C-143 PC to BWC with the examination report.
Who pays
The C-143 PC sets no fee and has no billing section; it is part of a BWC-arranged DEP examination.

Good to know

  • Each class ties to a share of an eight-hour day: Class 2 continuous (67 to 99 percent), Class 3 frequent, Class 4 occasional (1 to 33 percent), Class 5 never.
  • Abilities are grouped into activities of daily living, social function, thinking, concentration and judgment, and adaptation to stress, with 19 rated items in all.
  • Selecting Class 2, 3, 4 or 5 for any item requires a detailed explanation of work abilities, or of why the injured worker cannot perform any work.
  • The examiner states whether restrictions are temporary or permanent, the expected duration of temporary restrictions, and the hours per day and days per week the worker can work.
