Ohio BWC MEDCO-43: Caregiver Services Evaluation

In catastrophic Ohio workers' compensation claims, BWC relies on this treating physician evaluation to decide whether caregiver services should continue. The physician documents which activities of daily living need full or partial help, how often and for how long, along with mobility devices, hospital stays and a one-year prognosis. The form ends with a recommendation to continue or discontinue caregiver services.

Who completes it

The treating physician completes the whole form after examining the injured worker and signs it with a BWC provider number. No other party completes a section.

Good to know

  • BWC requires the form to be completed in its entirety at least once a year to avoid delays or denial of caregiver services.
  • ADLs rated include bathing, dressing, feeding, hygiene, taking medications, toileting and catheter or ostomy care, transfers, and transportation to medical appointments, each with times and hours per day.
  • New treatment needs found at the exam are not requested on this form; they go to the managed care organization on a C-9.
  • The physician notes conditions outside the allowed claim that contribute to caregiver needs, and whether decline is due to the allowed conditions or to aging and comorbidities.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often is MEDCO-43 required?

At least annually. The instructions say the treating physician must complete the form in its entirety at least once a year to avoid delays or denial of caregiver services and reimbursement. Office notes from the same exam should document medications, the current plan of care and any new services needed.

Can the physician recommend stopping caregiver services on MEDCO-43?

Yes. The certification section offers two choices: continue caregiver services, or discontinue them as of a stated date. The physician certifies that the recommendation is based on an examination of the injured worker, and the prognosis section asks for a rationale.
