NJ MVC Class K: Blind or Disabled Non-Driver ID Form

New Jersey's Motor Vehicle Commission charges blind and disabled people a reduced fee for a non-driver ID, and this one-page Class K certification is the proof. A physician explains the nature of the applicant's condition and signs before the applicant applies for an initial non-driver ID card. The card is identification only: it does not allow driving or parking in disability spaces.

Who completes it

The applicant prints their name, date of birth, address and any driver license number. A physician, podiatrist, chiropractic physician, PA or NP licensed in New Jersey or a bordering state completes the physician's section with license number and state of issuance, describes the condition, and signs.

Submitting and payment

How to submit it
The applicant brings the certification to an MVC agency with the license application (BA-208), or mails both to the MVC Government Unit for a renewal or duplicate.
Who pays
The applicant pays the reduced non-driver ID fee and nothing for renewals; MVC sets no fee for the physician's certification.

Good to know

  • The form must be printed, completely filled out and signed by the certifier before the applicant applies for an initial non-driver ID card.
  • The physician writes a free-text description of the applicant's condition; there are no checkboxes or listed qualifying conditions on the form.
  • The form itself states that the non-driver ID does not entitle the holder to park in a person with a disability parking space.
  • The current version is dated 1/19, and MVC's GU-12 instructions list which professions may certify.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign New Jersey's Class K non-driver ID certification?

MVC's instructions allow a physician, podiatrist, chiropractic physician, physician assistant or nurse practitioner licensed in New Jersey or a bordering state. The certifier enters their medical license number and the state that issued it, describes the applicant's blindness or disability, and signs the form.

Does the NJ Class K form give parking privileges?

No. The form states that a non-driver ID card is not a license to drive and does not entitle the holder to park in a person with a disability parking space. It only qualifies a blind or disabled applicant for the reduced-fee identification card.
