Colorado NEMT 25+ Mile Verification Form

Health First Colorado members who need non-emergent medical transportation (NEMT) to a provider more than 25 miles from home need this verification for the long-distance trip. It records the member, the medical facility and treating provider, and which of four listed reasons keeps the member from being seen within 25 miles. It is valid for 90 days from the initial visit to the treating provider.

Who completes it

Either the NEMT provider or the treating provider may complete it. It asks for the facility name and address, the treating provider's name and title, a contact phone and a Health First Colorado provider ID or NPI, then the printed name, date and organization of the person completing it.

Submitting and payment

How to submit it
The completed verification is submitted to the NEMT broker with the member's trip request.
Who pays
To be eligible for reimbursement, each NEMT transport must meet all applicable requirements of 10 CCR 2505-10, Section 8.014, on non-emergent medical transportation.

Good to know

  • The accepted reasons are: the closest provider will not accept the member, complex conditions keep the closest provider from accepting, the member moved within three months, or no provider is within 25 miles.
  • Validity runs 90 days from the date of the initial visit to the treating provider, and the form asks for that date.
  • Colorado has no general NEMT physician certificate; behavioral health secure transportation over 250 miles uses its own verification form.
  • Minors may also need the NEMT Consent and Liability Release for Minors, published in English and Spanish. The current form was updated April 2025.
