Health First Colorado Outpatient PAR Form

Health First Colorado providers request prior authorization (PAR) for outpatient services such as therapy, imaging, genetics labs, audiology, DME and supplies through Acentra Health, and this two-page form mirrors that request. Most providers must enter PARs in Acentra's Atrezzo portal; the PDF is only for new or revised PARs from providers approved to fax. It covers request type, providers, member, service, diagnoses and procedure lines.

Who completes it

The billing provider and the requesting or ordering provider are both identified by NPI or Health First Colorado ID; the requesting office completes the request, marking it new, a revision or a cancellation with the existing PAR number, and gives a contact name and phone.

Submitting and payment

How to submit it
Submit PARs electronically through the Acentra Health Atrezzo portal. Fax is allowed only for providers with an approved exemption requested on the Colorado Fax Exemption Form.

Good to know

  • Service types on the form include PT, OT, ST, personal care, genetics lab, imaging, pediatric behavioral therapy, audiology, wheelchairs, oxygen, orthotics and prosthetics, CRT, and oral, enteral and parenteral nutrition.
  • For DME, only member-owned equipment can be authorized for repair. Give the serial number, how long the item is needed in months and years, and its estimated cost.
  • List each procedure code separately with a narrative description, units requested and from and through dates, plus the medical necessity for the service and primary diagnosis codes.
  • Inpatient requests use a separate inpatient PAR form, and adult long-term home health has its own form. The outpatient form is revised August 2025.
