Good to know

  • Mark where the drug will be given: the member's home, a long-term care facility, the doctor's office, or a dialysis unit or hospital. Home health and facility administration add the agency or facility details.
  • Requests missing required information are delayed. Reviewing the criteria in Appendix P or the Preferred Drug List before filling in the form speeds approval.
  • The 04/2026 form reflects MedImpact, the pharmacy benefit manager effective 4/1/2026; Synagis keeps its own PA request form.
  • Members who qualify for Global Prior Authorization are exempt for one year from PA for non-preferred drugs, and step therapy exceptions use their own form.
