South Carolina Medicaid PA Request Form: Medications

South Carolina Healthy Connections Medicaid uses this universal form for pharmacy prior authorization, and it lists the fax line for fee-for-service Medicaid and for each managed care plan. A prescriber meets it when a member's prescription needs approval. The completed form goes to the member's plan, which reviews the drug history and the clinical rationale against medical necessity.

Who completes it

The prescriber or an authorized representative completes all five sections (provider, member, drug information, drug history for the diagnosis, and rationale) and signs.

Submitting and payment

How to submit it
Fax it to the member's plan using the plan fax numbers printed on the form, which cover fee-for-service Medicaid and each managed care plan.

Good to know

  • Use one form per member and one drug per form; incomplete, incorrect or illegible forms can delay the prior authorization process.
  • Plans listed include Absolute Total Care, Healthy Blue, First Choice by Select Health, Humana Healthy Horizons, Molina Healthcare and fee-for-service Medicaid.
  • Section IV asks whether the drug will be given in the office or taken home, and whether the request continues a previous approval.
  • Separate forms exist for antipsychotics in children 6 and under, adult and pediatric growth hormone, and hepatitis C treatment.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
