Delaware Medicaid Practitioner Medical Necessity Letter

Delaware Medicaid requires a practitioner's medical necessity letter for supplies and durable medical equipment that need prior authorization. Published as Appendix D of the Supplies and DME Provider Policy Manual, it asks the practitioner to describe diagnosis, prognosis, treatment plan, the reason for the item and its expected therapeutic effect. DMMA's Medical Review Team uses it to decide the supplier's request.

Who completes it

The physician or practitioner completes the whole letter and signs it with printed name, address and phone. The DME supplier attaches it to its prior authorization request.

Submitting and payment

How to submit it
Give the signed letter to the DME supplier, which attaches it to the prior authorization request reviewed by the DMMA Medical Review Team.
Who pays
The letter sets no fee for the practitioner; it supports the supplier's prior authorization for the item.

Good to know

  • A separate letter is required for each item requested, so a patient needing several items needs several letters.
  • Medicaid covers the least costly appropriate alternative that can be safely and effectively provided and does not duplicate other services, so explain why cheaper options will not work.
  • Attach pertinent laboratory or pulmonary function test results and summaries from other professionals involved in the member's care.
  • The Medical Review Team may request more information before deciding. The letter should state the estimated duration of use as well as the reason for the item.

How this list is checked

Frequently asked questions

Does Delaware Medicaid need one medical necessity letter per item?

Yes. The form states that a separate letter is required for each item requested. Each letter names the item and covers the diagnosis and prognosis, treatment plan, reason for use with estimated duration, and the expected therapeutic effect.

What does Delaware's Medical Review Team look for in the letter?

The team applies Medicaid's rule of covering the least costly appropriate alternative that can be provided safely and effectively without duplicating other services. A letter that ties the item to the member's functional limitations, includes test results and explains the expected effect gives the team what it needs; it may ask for more.
