Delaware Medicaid Comprehensive Medical Report (A)

Delaware's Division of Medicaid and Medical Assistance uses this one-page physician report to decide whether a long-term care applicant meets nursing home level of care, in a facility or in the community. The physician summarizes history, current impairments, test results, diagnoses, prognosis and treatment. It is returned with the Pre-Admission Evaluation to the long-term care medical eligibility unit.

Who completes it

The physician completes the whole report, from measurements and history to progress, then signs, dates and marks whether the applicant meets a nursing home level of care.

Submitting and payment

How to submit it
Return the completed report to DMMA's long-term care medical eligibility unit with the Pre-Admission Evaluation (PAE).
Who pays
DMMA lists no fee for the physician's report; it supports the applicant's long-term care eligibility.

Good to know

  • A note to the physician asks for a complete and legible report so the reviewer can judge the nature and severity of impairment accurately.
  • Objective findings should include X-ray, lab and other diagnostic results with dates; a separate sheet can be used if needed.
  • Progress is marked as improving, static, deteriorating or terminal, and treatment covers therapy and response, visit frequency, last exam date, diet, medications and recommended activities.
  • Related DMMA forms include Comprehensive Medical Report (B), the Master PAE, the PASARR Exemption Certificate and the LTC Awareness Form. The current revision is 10/20/2017.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
