Missouri Physician Certification for Personal Care

Agencies delivering personal care to MO HealthNet children under 21 send this certification to the child's physician before they can request prior authorization from the Bureau of Special Health Care Needs. The agency fills in a care plan describing the amount, duration and scope of services, and the physician certifies that those services are medically necessary for a patient under their care.

Who completes it

The agency enters its details, the physician's, the child's name, Medicaid number, birthdate and diagnosis, and the care plan, and an agency representative signs. The physician signs and dates the certification.

Submitting and payment

How to submit it
Return the signed certification to the agency that sent it; the agency then requests prior authorization for the personal care services from the Bureau of Special Health Care Needs.

Good to know

  • It is not a standard form. The agency may create its own version, as long as it contains sufficient information, or copy MO HealthNet's sample, so layouts vary.
  • By signing, the physician also states that they will review the child's need for personal care at least every six months.
  • The certification covers the services exactly as described in the agency's care plan, so the amount, duration and scope should match what the physician considers necessary.
  • The sample carries no form number on the MO HealthNet forms index. Children's personal care also uses form 2575-072, the Personal Care Plan for Children.
