MO HealthNet Prior Authorization Request 2575-034

MO HealthNet uses form 2575-034 to request fee-for-service prior authorization for medical services, durable medical equipment, prostheses and Healthy Children and Youth (HCY, Missouri's EPSDT) services. Providers meet it when a service needs approval before delivery. A state consultant reviews up to 12 service lines, approves or denies each one, and sets the allowed amount for manually priced codes.

Who completes it

The requesting provider completes Sections I to IV and signs where the provider manual requires. The prescribing or performing practitioner completes and signs Section V for prescribed services such as DME or physical therapy, or when the requester is a clinic or group; signature stamps are not accepted.

Submitting and payment

How to submit it
Mail the form with supporting documents to the address on the form, using cover sheet 2575-044 for DME documentation, or submit through the eMOMED portal. Missing or invalid required fields send the request back before review.
Who pays
Authorization approves medical necessity only; it does not guarantee payment or that the billed amount will be reimbursed, and the participant must be eligible on the date of service or delivery.

Good to know

  • Starred fields are required: the 8-digit MO HealthNet ID, procedure codes and modifiers, from and through dates, quantity, and the amount to be charged for manually priced codes.
  • Section II applies to HCY (EPSDT) requests: give the screening date, whether it was full, inter-periodic or partial, and the screening provider. A plan of care may be required.
  • Section V asks the date the disability began and the expected months of medical need, so DME and therapy prescribers should have those ready.
  • Write the medical necessity explanation in field 25 and attach pages if needed rather than using another PA form. The current version is dated February 2026.
