Pennsylvania MA 97 Outpatient Services Authorization

Pennsylvania's Department of Human Services uses the MA 97 to request fee-for-service Medical Assistance prior authorization, or a program exception under the 1150 waiver, for outpatient services, equipment and supplies. A practitioner meets it when prescribing or requesting a service that needs approval for a fee-for-service beneficiary. The DHS copy goes to the outpatient prior authorization and 1150 waiver unit, which issues a notice.

Who completes it

The requesting provider completes the form from eligibility and PROMISe enrollment data, and the referring practitioner or prescriber signs item 35 attesting medical necessity. The patient or an authorized representative must also sign item 33.

Submitting and payment

How to submit it
Send the DHS copy to the Outpatient PA/1150 Waiver Services unit with a copy of the prescription when the service requires one, and keep the provider copy; DHS also accepts requests by fax and through its prior authorization portal.
Who pays
For 1150 waiver requests, the provider enters the exact amount per unit, quantity per month and number of months requested.

Good to know

  • Check only one box, prior authorization or 1150 waiver; if both are needed, complete separate MA 97s.
  • Item 29 must give a narrative of the symptoms the service is meant to relieve and enough medical justification for every requested item.
  • Mental health requests need a current psychological or psychiatric evaluation, treatment plan and plan of care summary from the prescribing professional.
  • For a resubmission, check item 32 and enter the reference number from the denied Prior Authorization Notice or Program Exception Notice.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the patient have to sign the Pennsylvania MA 97?

Yes. Item 33 must be signed by the patient or an authorized representative, with the date in item 34. The practitioner or prescriber requesting the service signs item 35, or uses a signature stamp, attesting the service is medically necessary and the information is accurate.

Do HealthChoices members use the MA 97?

No. The MA 97 is for fee-for-service Medical Assistance. Beneficiaries enrolled in a HealthChoices managed care organization go through that plan's prior authorization process, so confirm enrollment through the Eligibility Verification System before completing items 3 to 6.

What goes in item 27 of the MA 97?

Item 27 asks what other alternatives have been tried or used to meet the patient's needs. Attach documentation of those alternatives and why they did not work. If no alternatives have been tried or used, enter N/A rather than leaving it blank.
