Pennsylvania MA 372 Certification of Terminal Illness

Pennsylvania Medical Assistance uses this one-page certification so a recipient can elect the hospice benefit. The physician writes the diagnosis and its ICD-CM code and states that, in their professional opinion, life expectancy is six months or less. The form marks whether it is an initial certification or a recertification and has three dated physician signature lines.

Who completes it

The patient's attending physician and the hospice medical director sign, and a third line is for the interdisciplinary team physician. Each signature has its own date.

Submitting and payment

How to submit it
Return the signed form to the hospice, which keeps it with the recipient's hospice election statement.

Good to know

  • Both the written diagnosis and the ICD-CM diagnosis code are numbered fields, along with the recipient number and name.
  • The same form handles initial certification and recertification; check the matching box each time it is completed.
  • The recipient's own hospice election is documented separately on form MA 341, the Recipient Statement.
  • The current version is MA 372 6/16, posted on the DHS Medical Assistance provider forms page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who signs the Pennsylvania MA 372?

The form has three signature lines: the patient's attending physician, the hospice medical director and the interdisciplinary team physician, each with a date. Together they certify the written diagnosis and that the recipient's life expectancy is six months or less in their professional opinion.

Is MA 372 used for hospice recertification?

Yes. The form has checkboxes for initial certification and recertification, so the same one-page certification is completed again when the hospice recertifies. The hospice keeps each signed form with the recipient's election documentation.
