MaineCare Hearing Aids Prior Authorization Form

MaineCare requires prior authorization for hearing aids, and the Office of MaineCare Services publishes a two-page form that sets out the clinical criteria and submission routes. Audiologists, hearing aid dealers and ordering providers meet it when a member needs a monaural or binaural aid. Requests are reviewed under MaineCare Benefits Manual Chapter II, Section 60, which covers medical supplies and durable medical equipment.

Who completes it

The audiologist or ordering provider prepares the request and documents the criteria: an otologic evaluation by a primary care provider or otolaryngologist and a clinical audiology evaluation showing the need for amplification. The member completes nothing.

Submitting and payment

How to submit it
Submit through the Health PAS Online Portal (the MaineCare Provider Web Portal), or by telephone, fax or mail to the Office of MaineCare Services Prior Authorization Unit. Items without a MaineCare fee schedule rate also need the Supplemental DME Prior Authorization Form (MA56).

Good to know

  • Members 21 and over need documented moderate to severe hearing loss, 41 to 90 dB HL, in each ear for which a hearing aid is requested.
  • Members under 21 must receive a hearing aid orientation covering use and care, counseling on expectations and limitations, and ancillary needs such as auditory rehabilitation.
  • Hearing aids must be bought from a licensed audiologist or hearing aid dealer and fitter using a vendor contracted through the State of Maine hearing aid procurement program.
  • MaineCare is fee-for-service without managed care plans, so these criteria apply to every member. The current version is dated July 2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
