Texas Medicaid F00144: Oxygen Therapy PA Request

Texas Medicaid uses TMHP form F00144 to prior authorize home oxygen therapy devices and supplies. A physician or allowed practitioner meets it when ordering oxygen, because Section B must be completed by the prescriber with the qualifying arterial blood gas or oxygen saturation results. The DME provider submits the request to TMHP for initial approval and renewals.

Who completes it

The rendering DME provider may complete Section A and signs a statement that the supplied items match the prescriber's order. The physician or allowed practitioner must complete and sign Section B, the oxygen therapy request and medical necessity documentation.

Submitting and payment

How to submit it
Submit through TMHP's PA on the Portal or by fax; faxed requests can take up to 24 hours to reach the PA Department.
Who pays
The submitter certification states that prior authorization is a condition of reimbursement, not a guarantee of payment.

Good to know

  • Record the testing date and the lowest saturation or arterial pO2 at rest or with exercise and during sleep.
  • For initial requests with pO2 of 56 to 59 mm Hg or saturation of 89 percent or higher, note dependent edema, cor pulmonale or erythrocythemia with hematocrit.
  • Renewal requests ask whether the client was compliant with oxygen use as ordered in the initial request; supplies-only requests certify the client owns the device.
  • Duration of need and the date the client was last seen by the prescriber must be filled in, or the request is returned.
