Texas Medicaid F00028: Home Health Plan of Care (POC)

Texas Medicaid uses form F00028, the Home Health Plan of Care, to prior authorize home health skilled nursing and home health aide visits, therapy, supplies and DME through TMHP. A physician or allowed practitioner meets it when signing the plan of care for a client receiving home health services. The home health agency submits it, and TMHP reviews the orders and medical necessity narrative.

Who completes it

The home health agency fills in the client, agency, PPECC and plan of care details, and the agency RN signs. The physician or allowed practitioner provides diagnoses, orders and medical necessity, then signs the plan of care and the conflict of interest statement.

Submitting and payment

How to submit it
Submit through TMHP's PA on the Portal, which reaches the PA Department immediately, or by fax, which can take up to 24 hours to be scanned and entered.
Who pays
The submitter certification states that prior authorization is a condition of reimbursement and not a guarantee of payment.

Good to know

  • The physician certifies no significant ownership interest in, or financial relationship with, the billing home health agency, unless the governmental or sole community agency exception under 42 CFR 424.22 applies.
  • Section A asks for the date the client was last seen by the physician or allowed practitioner.
  • Each DME item is marked own, repair, buy or rent, with how long it is needed.
  • Related TMHP forms include the Home Health PA Checklist (F00029) and the private duty nursing PA form packet (F00071).

What clinical information goes on the Texas Home Health Plan of Care?
