Texas Medicaid F00012: CCP Prior Authorization Request

Texas Medicaid's Comprehensive Care Program (CCP) covers medically necessary services for clients under 21, and form F00012 requests TMHP prior authorization for CCP services such as applied behavior analysis, DME, medical supplies, private duty nursing, PPECC and inpatient rehabilitation. A physician meets it when certifying the request in Section F. The provider submits it through TMHP's PA on the Portal or by fax.

Who completes it

The rendering provider, supplier or qualified rehabilitation professional completes Sections A to E, including the request type, diagnosis and medical necessity, and HCPCS lines. The requesting physician or allowed practitioner completes and signs Section F, the Primary Practitioner's Certifications.

Submitting and payment

How to submit it
Submit through TMHP's PA on the Portal for a faster decision, or fax the form; fax requests are scanned and data-entered first, which can take up to 24 hours.
Who pays
The submitter certification states that prior authorization is a condition of reimbursement, not a guarantee of payment, and claims are paid from federal and state funds.

Good to know

  • Signatures from chiropractors and PhDs are not accepted, and signature or date stamps are not acceptable on Section F.
  • Nurse practitioners, physician assistants, clinical nurse specialists and certified nurse midwives may sign for ABA, private duty nursing and therapy services when the physician delegates that authority.
  • Fields marked with an asterisk are essential; leaving one blank gets the request returned, and any other incomplete portion can pend it.
  • A revision cannot extend beyond the current authorization period; HCPCS codes with descriptions, quantity and frequency are required in Section E.
