Texas Medicaid F00049 Physician's Examination Report

Texas Medicaid uses this one-page report for the physician's ear examination and medical clearance before a client is evaluated and fitted for a hearing aid. The physician records ear findings and cerumen removal, whether more otolaryngology care is needed, any medical contraindication by ear, and whether the client is a hearing aid evaluation candidate. The patient takes the signed original.

Who completes it

A physician or delegate completes the examination and physician sections and signs; stamped signatures are not accepted. If more otolaryngological evaluation is needed, the patient is referred for a consultation and completion of this form.

Submitting and payment

How to submit it
Give the patient the signed original and one copy for the hearing aid provider. To be reimbursed for the exam, the physician also submits a completed form with the claim for physician's services to TMHP.
Who pays
TMHP reimburses the examination when the completed form is submitted together with the physician's services claim.

Good to know

  • A yes to question 3 means the patient needs further otolaryngological examination or treatment before medical clearance and should be referred for consultation.
  • Question 4 records medical contraindications to hearing aid use and lets the physician mark the ear, right or left, where a hearing aid is medically prohibited.
  • The physician or delegate prints name, medical specialty and contact details, and the form was revised 12/12/2018.
