Wisconsin Medicaid PA/POR F-11019: Otological Report

Wisconsin Medicaid uses this one-page physician report to support prior authorization of a hearing instrument. It is mandatory when a hearing instrument specialist requests PA, and audiologists may use it in place of a physician prescription kept in the member's record. The physician documents hearing history and ear findings and refers the member for a hearing instrument evaluation.

Who completes it

The requesting physician completes all three sections: provider information with NPI, member information, and the documentation section covering hearing loss history, otological findings, diagnosis and recommendations. The physician signs and enters the date the request is made.

Submitting and payment

How to submit it
The physician gives one copy to the member to take to the testing center and keeps a second copy. The hearing provider attaches it to the PA/RF with the PA/HIAS2 hearing instrument request sent to ForwardHealth by fax or mail.

Good to know

  • Element 11 records canal, ear drum and middle ear findings for each ear as normal or a described problem, and Element 12 captures added studies such as caloric and postural tests.
  • The physician must refer to an audiologist if the member is 21 or under, is behaviorally or cognitively impaired, or has other special needs requiring a comprehensive evaluation or specialized diagnostic tools.
  • If none of those situations applies, either an audiologist or a hearing instrument specialist may provide the evaluation. A separate checkbox flags when a home hearing test is required.
  • Completion instructions are in F-11019A. Use the Enrollment Verification System spelling of the member's name, and enter only the member ID number in Element 8.

Frequently asked questions

When is the PA/POR required in Wisconsin?

The F-11019A instructions say the PA/POR is mandatory when a hearing instrument specialist requires prior authorization for a hearing instrument. Audiologists may use it in place of a physician prescription, which they keep in the member's medical record. It supports the PA/HIAS2 hearing instrument request.

Who must evaluate a child for a hearing aid under Wisconsin Medicaid?

In Element 15 the physician refers members aged 21 or under to an audiologist for the hearing instrument evaluation, as BadgerCare Plus regulations require. The same audiologist referral applies to members who are behaviorally or cognitively impaired or have other special needs requiring specialized diagnostic tools.

What does the physician sign on the PA/POR?

The requesting physician signs at the bottom of Section III and enters the date the request is made. Above the signature, the physician records the clinical diagnosis of hearing status, describes any medical, cognitive or developmental problems, and checks the applicable recommendation boxes.
