Wisconsin F-01197 Certification of Need for SMV

Wisconsin Medicaid requires this certification before a member can use specialized medical vehicle (SMV) transportation to get to medical care. The evaluator confirms that a physical or mental impairment makes travel by mass transit, taxi or private vehicle unsafe, classifies the member as indefinitely disabled, legally blind or temporarily disabled, and explains why an SMV is needed.

Who completes it

Only a physician, physician assistant, nurse midwife or nurse practitioner may evaluate the member, complete Sections III and IV and sign with an original signature. The SMV provider gives the blank form to the member and keeps the completed original.

Submitting and payment

How to submit it
Give the signed original to the member and keep a copy; the SMV provider must receive it within 14 working days of the signature date.

Good to know

  • Validity depends on the box checked: indefinitely disabled or legally blind lasts 36 months from signing; temporarily disabled lasts no more than 90 days and can be renewed.
  • If the member can travel safely by common carrier, stop at Section II, do not sign, and refer them to the Medicaid transportation coordinator at their county or tribal agency.
  • Stamped or photocopied signatures, altered forms and alternate versions are not accepted, and every element must be completed.
  • Completion instructions are in F-01197A, which also defines indefinitely disabled under DHS 107.23(1)(c)1, Wis. Admin. Code.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Wisconsin SMV certification of need?

Only a physician, physician assistant, nurse midwife or nurse practitioner may act as the evaluator and sign F-01197. The signature must be original, not stamped or photocopied, and the evaluator must have evaluated the member and found that they cannot travel safely by common carrier.

How long is a Wisconsin F-01197 valid?

For a member certified as indefinitely disabled or legally blind, it is valid for three years (36 months) from the date the evaluator signs. For a temporarily disabled member, it is valid for no more than 90 days, with the expected duration stated if shorter, and it may be renewed.

Does the SMV provider have a deadline to receive F-01197?

Yes. The completion instructions tell the SMV provider to accept the form only if it is received within 14 working days of the date the evaluator signed it. The provider keeps the completed original in the member's file for five years from the last date of service billed under it.
