Wisconsin Medicaid PA/ENFA F-11054: Enteral Formula

Wisconsin Medicaid requires this four-page attachment when enteral nutrition formula needs prior authorization. It captures caloric requirements, how formula is given, diagnoses, height and weight, and why table foods or general-purpose formula are not enough. Section VII converts calories or milliliters into units requested, and the billing provider signs before the request goes to ForwardHealth with the PA/RF.

Who completes it

Clinical content comes from the prescriber, often with a dietitian, and the prescriber's name and NPI appear in Section II. Section IX is signed by the billing provider, who certifies that clinical information submitted with the request was obtained from the prescriber.

Submitting and payment

How to submit it
Attach it, with a copy of the prescription or order, to the PA/RF and submit it to ForwardHealth through the Portal, by fax or by mail. Completion instructions are in F-11054A.

Good to know

  • Submit a copy of the prescription or order with each request; the form says prescriptions or orders should not be more than one year old.
  • If the member receives less than 50 percent of daily nutrition orally from a complete formula, describe the plan to decrease dependence or why decreasing it is not possible.
  • Tube-fed-only members skip Elements 15 through 17. For failure to thrive or malnutrition, Element 19 asks for anthropometrics such as growth-chart progression or BMI, plus supporting lab values.
  • Units are calculated on the form: calories per day times days, divided by 100, or for milliliter-based codes, milliliters per day times days, divided by 500.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who signs the Wisconsin PA/ENFA?

Section IX carries the authorized signature of the billing provider, with printed name, position title and date. By signing, the billing provider attests to the accuracy and completeness of the request and that clinical information submitted with it, such as medical records, was obtained from the prescriber.

Which medical conditions does the PA/ENFA list?

Element 15 lists inborn errors of metabolism, conditions requiring specialized oral nutrition such as a ketogenic diet or eosinophilic esophagitis, impaired absorption, central nervous system disease interfering with ingestion, nutritional deficiency, chronic disease such as end-stage renal disease, ongoing cancer treatment, and other.

How old can the enteral formula prescription be?

Section III asks for the date the prescription or order was written and says it should not be more than one year old. A copy must be submitted with each PA request, and Element 16 asks for any clinical changes since previously approved requests.
