Montana DPHHS-SLTC-86 Level-of-Care Determination

Montana's Senior and Long Term Care Division uses this screen to decide whether an applicant meets nursing facility level of care, for nursing facility admission or for home and community based services (HCBS). The referring hospital, facility or case manager records diagnoses, treatments and a coded functional assessment. Mountain-Pacific Quality Health reviews it and records the determination in its own section.

Who completes it

The referring facility, hospital clinician or case manager completes the identifying, diagnosis and functional assessment portions and names the applicant's health care professional. The Mountain-Pacific Quality Health sections are completed only by the reviewer.

Submitting and payment

How to submit it
Send the completed screen to Mountain-Pacific Quality Health for review. The reviewer's determination is copied to the case management team, the nursing facility and the referral source.

Good to know

  • Mark the program requested at the top: Nursing Facility, HCBS (Initial), HCBS Yes/Discretionary, or Unknown. The same screen serves both settings.

  • The mental status section covers orientation, memory loss, impaired judgment, danger to self or others and whether 24-hour supervision is needed, plus respiratory status and ability to summon emergency help.
  • Nursing facility referrals also involve the separate DPHHS-SLTC-145 Nursing Facility Level I Screen; the reviewer records the Level I date on this form.
