Montana Medicaid CMN: Motorized Wheelchair

Montana Medicaid asks the treating physician to certify medical necessity on this one-page certificate when a supplier requests a motorized wheelchair. Its questions cover current wheelchair ownership, residence, daily hours of use, inability to use a manual chair, safe operation, ambulation and home access, followed by a narrative of every item and option. The supplier attaches it to the DME prior authorization request.

Who completes it

The treating physician answers the questions, describes the equipment and signs the certification. The supplier, often working with a therapist, gathers the item details and sends the certificate with its prior authorization request.

Submitting and payment

How to submit it
Return the signed certificate to the DME supplier, which attaches it to the Montana Medicaid DME Prior Authorization Form.

Good to know

  • Signature and date stamps are not acceptable; the treating physician must personally sign and date the certification in mm/dd/yyyy format.
  • Continuation sheets for the item narrative are allowed if they carry the patient and physician information at the top and the physician's signature.
  • Estimated length of need is entered in months from 1 to 99, where 99 means lifetime, along with diagnosis, prognosis, height and weight.
  • Montana uses item-specific certificates: manual wheelchairs, hospital beds, prosthetics and orthotics, and parenteral therapy each have their own DME CMN. This one is revised October 2014.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
