Wyoming Medicaid Documentation of Medical Necessity

Wyoming Medicaid uses this one-page form for a physician or other provider to summarize the history, examination, laboratory and imaging findings, and prescriptions that show a prescribed service is medically necessary. It includes a space to justify services performed out of state. The agency records its approval on the same page, though approval does not guarantee payment.

Who completes it

The physician or other provider completes the member, provider and service sections and signs the attestation. The approval section at the bottom is for agency use only.

Submitting and payment

How to submit it
Mail the completed form to the Wyoming Medicaid fiscal agent, as printed on the form, or attach it to the prior authorization or claim using the Wyoming Medicaid Attachment Coversheet.

Good to know

  • Item 13 must give enough detail from the exam, labs, X-rays, prescriptions and other records to satisfy medical necessity; extra documentation can be attached.
  • Item 15 is for services performed out of state and asks for a brief justification statement explaining why the service was provided outside Wyoming.
  • Approval is subject to the member's eligibility and Wyoming benefit limits, and the form reminds providers to confirm the identification card is current before service.
  • Wyoming Medicaid posts separate forms for services with their own paperwork, such as the Abortion Certification Form and the Hysterectomy Acknowledgment of Consent Form.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
