DC Medicaid Form 719A: Prior Authorization Request

The District of Columbia's Department of Health Care Finance uses Form 719A for prior authorization in its fee-for-service Medicaid program. One page covers surgery, DME, medical, pharmacy, dental, eyewear, hospice, home health and private duty nursing requests, with diagnosis and procedure codes, units and justification. It applies only to fee-for-service members, since most DC Medicaid enrollees belong to managed care organizations.

Who completes it

The requesting provider completes the patient, prescribing provider and servicing provider blocks, the requested services and justification, then signs the certification that the services are medically necessary. For DME, home health and private duty nursing, the requesting physician also certifies the date of a face-to-face encounter.

Submitting and payment

How to submit it
Send the form with supporting documentation to DHCF's fee-for-service prior authorization and utilization management contractor.
Who pays
Each service line asks for estimated charges alongside the diagnosis code, procedure code, time required and frequency or units requested.

Good to know

  • The face-to-face certification names the allowed prescriber, who may be a primary physician, nurse practitioner, certified nurse midwife, physician assistant, or acute or post-acute physician, plus the encounter date.
  • A checklist flags DME needing a face-to-face encounter: TENS units, rollabout chairs, cervical traction, oxygen and respiratory equipment, hospital beds, items above a fee schedule price ceiling, and CMS Specified Covered Items.
  • Dental requests use the printed tooth chart: mark missing teeth with X, teeth to be extracted with a question mark, and teeth already x-rayed with V.
  • The current version is dated June 2018. DHCF lists the Request for Determination of Coverage and Pricing and the DME MADS Referral Form as related forms.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does Form 719A apply to DC Medicaid managed care members?

No. The 719A belongs to the District's fee-for-service Medicaid program. Most DC Medicaid members are enrolled in managed care organizations, so confirm the patient's coverage first; a managed care member's prior authorization goes through that plan rather than through this DHCF form.

Who can be the face-to-face prescriber on the 719A?

The DME and home health certification lists a primary physician, nurse practitioner, certified nurse midwife, physician assistant, or acute or post-acute physician. The requesting physician records the encounter date and the prescriber's name and title, certifying that the encounter related to the primary reason the patient needs the DME or home health services.

What does the requesting provider certify by signing Form 719A?

That the requested services are medically indicated and necessary for the patient's health, and that the information on the form is true, accurate and complete. The signature block also asks for the provider's title and the date of signing.
