Unum CL-1064 Voluntary Benefits Disability Claim APS

Unum uses this 11-page form for voluntary benefits (worksite) disability claims, voluntary life waiver of premium, or both. In the Attending Physician Statement on pages 7 to 9, the patient completes Part I and the physician completes Part II. Part II covers normal pregnancy dates or the diagnosis and findings for other conditions, plus treatment, a functional capacity grid and a return to work assessment.

Who completes it

The physician or treating provider primarily responsible for care completes and signs Part II after the patient fills in Part I. The employee completes the Employee Statement on pages 4 to 6 and signs the authorization.

Submitting and payment

How to submit it
The provider faxes or mails Part II to Unum's Benefits Center using the contacts printed on the form.
Who pays
The instructions state that Unum is not responsible for expenses associated with completing the physician statement.

Good to know

  • For a normal pregnancy, complete only section A (expected and actual delivery dates, delivery type, first visit, stop-work date) and then go to section C.
  • The functional capacity grid rates sitting, standing, walking, climbing, reaching, fine finger and hand-eye movements, and lifting from up to 10 lbs to 100 lbs.
  • The diagnosis section still prints DSM-IV multi-axial codes alongside the ICD code, and asks about cognitive deficits or psychiatric conditions that affect function.
  • A bilingual English and Spanish edition is CL-1064-BL, and Unum's forms library also lists CL-1069 for the same PDF.
