Unum CL-1021 Disability Status Update Physician Form

Unum uses this 11-page update to re-evaluate whether a claimant still qualifies for disability benefits on an open claim. The physician or treating provider primarily responsible for care completes the Attending Physician Information on pages 8 to 10: current diagnosis with ICD and DSM codes, other treaters, functional capacity, treatment and recent or planned procedures.

Who completes it

The physician or treating provider completes sections A to E of the Attending Physician Information and signs section E. The employee completes the Employee Information on pages 4 to 6 and signs the authorization on the last page, giving a copy to the physician.

Submitting and payment

How to submit it
The provider faxes or mails the completed pages to Unum's Benefits Center, using the fax number and address printed on the form.
Who pays
The form lists no fee, billing code or Unum payment for completing the physician section.

Good to know

  • Unum warns that answers such as 'no work' or 'totally disabled' will not let it evaluate the claim and may lead to a call for clarification.
  • Quantify terms like prolonged or repetitive against an 8-hour workday: occasional means under 33% of the time, frequent 34 to 66%, constant 67 to 100%.
  • If the patient has no physical or behavioral health restrictions or limitations, initial section C and go to section E.
  • Attach supporting reports such as office notes, medical records, medication logs, consultations or testing. Related Unum forms include CL-1019 and the CL-1088 authorization.

Frequently asked questions

Why is Unum asking me to complete CL-1021?

Unum sends the Disability Status Update to collect current information used to evaluate the claimant's continued eligibility for disability benefits. The employee completes their section and asks the physician or treating provider primarily responsible for their care to complete the Attending Physician Information.

How should restrictions be described on Unum CL-1021?

List physical and behavioral health restrictions (activities the patient should not do) and limitations (activities the patient cannot do) separately, each with from and to dates. Be specific and quantify terms against a standard 8-hour workday with breaks about every two hours.

What treatment questions are on the Unum status update?

Section D asks for your treatment plan including all medications, any procedures or hospitalizations since you last reported, with dates, and any procedures expected in the next three months. Section A also records height, weight, blood pressure and the next office visit date.
