Unum uses this 15-page form to open a claim under an individual disability income policy. The insured supplies an individual statement, an occupation description and a signed authorization.

Who completes it

The physician or treating provider completes sections A to F of the Attending Physician Statement and signs section F. The insured completes the Individual Statement (pages 4 to 7), the Occupation Description Statement (pages 8 to 10) and the authorization.

Submitting and payment

How to submit it
Fax or mail the statement to Unum's individual disability claims unit using the fax number and address on the form, which differ from Unum's group claims lines.
Who pays
The form lists no fee, billing code or Unum payment for the physician statement.

Good to know

  • Section A is for a normal pregnancy only (delivery dates and type, first date unable to work); all other conditions use section B.
  • Physical and behavioral health restrictions and limitations each have their own box with from and to dates; 'no work' or 'totally disabled' will not let Unum evaluate the claim.
  • Section D asks whether you expect improvement in the patient's functional abilities and, if so, when.
  • Related forms are CL-1088, the HIPAA authorization for disability claims, and CL-1021, the Disability Status Update used on continuing claims.
