Unum CL-1234 Group Life Waiver of Premium Claim

Unum uses this 18-page form when an employer and employee ask that group life insurance premiums be waived because the employee is totally disabled. The Attending Physician Statement on pages 7 to 9 documents diagnosis, functional capacity, treatment and other treating providers.

Who completes it

The physician or treating provider primarily responsible for care completes sections A to D and signs section D. The employer completes the Employer Statement on pages 4 to 6, and the employee completes the Employee Statement, a work experience questionnaire and the authorization.

Submitting and payment

How to submit it
The provider faxes or mails the completed statement to Unum's Benefits Center using the fax numbers and address printed on the form.
Who pays
The form lists no fee, billing code or Unum payment for the physician statement.

Good to know

  • Proof of total disability must reach Unum within the time frames the policy specifies after the employee's date of loss.
  • Section A asks whether you advised the patient to stop working, whether the condition is work related, hospitalization dates, and any surgery with its CPT code.
  • Answers like 'no work' or 'totally disabled' do not let Unum evaluate the claim; quantify restrictions and limitations against an 8-hour workday.
  • Related Unum forms include CL-1088, the claim authorization, and CL-1212, the optional authorization to share information with third parties.
