Colonial Life 08727 Universal Claim Form

Colonial Life policyholders with more than one policy use this 11-page form to claim accident, cancer, critical illness, disability, pregnancy or hospital benefits for one event. The physician's parts cover hospital or rehabilitation confinement (Section 5A), surgery or diagnostic procedures (Section 5B) and, for disability or sickness, the Section 7 physician statement with diagnosis, restrictions and unable-to-work dates.

Who completes it

The policy owner completes the claimant, accident and confinement sections, and the employer completes Section 4 for a disability claim. The treating physician completes Sections 5A, 5B and 7 as they apply and signs each one.

Submitting and payment

How to submit it
The policyholder files online, where documents can be uploaded, or faxes or mails the paper form to Colonial Life. The form's checklist says the physician and employer must sign and date their own sections.
Who pays
The form names no fee for the physician's sections; for a patient covered by Medicaid, most non-disability benefits are assigned and paid to Medicaid or to the medical provider.

Good to know

  • An itemized hospital bill with admission and discharge dates and daily room charges can replace the confinement section; for surgery, send the surgeon's itemized bill with procedure codes and the operative report.
  • Sections 5A, 5B and 7 each ask for every date the patient received advice, diagnosis or treatment for the condition, or a related one, in the 18 months before it began.
  • For disability, the physician gives full-time and part-time unable-to-work dates, house confinement, activities of daily living the patient cannot perform, and permanent limitations versus restrictions.
  • Revision 5-26. Each physician block asks whether the practice accepts the standard HIPAA release and record requests by fax, and the fraud warning covers attending physician portions.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Which sections of Colonial Life's universal claim does the doctor complete?

Section 5A covers hospital, observation, intensive care or rehabilitation confinement with admission and release times. Section 5B covers inpatient or outpatient surgery and diagnostic procedures with codes and follow-up visits. For a disability or sickness claim, the physician also completes the Section 7 physician statement, including diagnosis, treatment plan and unable-to-work dates.

Can itemized bills replace the physician sections on form 08727?

Often, yes. For hospital or rehabilitation confinement, Colonial Life accepts an itemized bill showing admission and discharge dates and daily room charges. For surgery or a diagnostic procedure, it accepts the surgeon's itemized bill with procedure codes plus the operative report. A disability claim still needs the physician's statement with diagnosis, treatment and unable-to-work dates.

What pregnancy details does the universal claim ask the physician for?

Section 5A and Section 7 both carry a pregnancy block: date first treated, delivery date, vaginal or C-section delivery, surgical procedure code, anesthesia, and newborn admission and discharge dates. Section 5A also asks about births outside a hospital, and Section 7 asks for the diagnosis behind any high-risk pregnancy complications.
