Colonial Life 46988: Continuing Disability Claim

Colonial Life uses this six-page form for each later period of an open accident or sickness disability claim; the form itself says not to use it for a first claim. The employer confirms work status in Section 2, and the treating physician updates the medical picture in Section 3, including unable-to-work dates and an expected return to work.

Who completes it

The policy owner completes Section 1, naming the condition that keeps them from working, and the employer completes Section 2. The treating physician completes and signs Section 3, the physician statement.

Submitting and payment

How to submit it
Once the employer and physician have completed their sections, the claimant can upload the form to the existing claim online, or fax or mail it to Colonial Life.

Good to know

  • Section 3 asks how soon significant improvement is expected (1 to 2, 3 to 4, 5 to 6, or more than 6 months) and whether any restrictions or limitations are permanent.
  • Tests and surgeries are listed with dates and CPT codes, and copies of the test results and operative reports go with the claim.
  • The physician also records house confinement dates, activities of daily living the patient cannot perform, and hospital and office visit dates from the last 3 months.
  • A first injury or sickness goes on Colonial Life's Disability Claim, form 64387; form 46988 only continues an open claim. The current revision is 9-25.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is Colonial Life's continuing disability claim used?

Only on an open claim. The form states it must not be used when filing for an injury or sickness for the first time; that claim goes on Colonial Life's Disability Claim, form 64387. Form 46988 updates each later period with the employer's work status and the physician's current findings.

What does the physician complete on Colonial Life form 46988?

Section 3, the physician statement: the diagnosis preventing work and any secondary diagnoses, symptoms and treatment plan, tests and surgeries with CPT codes, last and next visit dates, full-time and part-time unable-to-work dates, expected return to work, house confinement, activities of daily living, and permanent limitations or restrictions.

Does the employer have to fill out Colonial Life form 46988?

Yes.
