Aflac Group Life Waiver of Premium Claim

Continental American Insurance Company uses this nine-page packet when an employee with Aflac Group life insurance becomes disabled and asks to have premiums waived. The attending physician's statement on pages 8 and 9 covers history, diagnoses with codes, objective findings, treatment dates, progress, functional capacity and a return-to-work plan. The employee, employer and physician each complete their own statement.

Who completes it

The employee completes the Employee's Statement and signs the release authorization, and the employer completes the Employer's Statement. The treating physician completes the Attending Physician's Statement and returns it to the patient.

Submitting and payment

How to submit it
The physician returns the completed statement to the patient, who sends all statements to Aflac's claims team by email, fax or mail as listed in the packet's filing instructions.
Who pays
The form states the physician statement must be completed without expense to the insurance company.

Good to know

  • The packet is a flat PDF with no fillable fields, so the physician statement is completed on a printed copy.
  • Item 6 rates sedentary, light, medium and heavy activity in an 8-hour day, how often the patient can bend, climb or reach, and upper extremity functions such as grasp and fine manipulation.
  • Item 7 rates mental health ability on a five-level scale, and item 5 records cardiac functional capacity using American Heart Association classes.
  • The return-to-work plan asks whether a plan was discussed, whether the patient is motivated, the release date and hours, and recommended job modifications.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
