Good to know

  • For pregnancy-related claims, Part C asks whether the patient has delivered, the delivery date and method, and any complications that would extend the disability.
  • The physician rates the patient as recovered, improved, unchanged or retrogressed and estimates when a fundamental or marked change is expected, from one month to never.
  • Part C asks whether the disability relates to the patient's job or any other work, and whether the patient can perform regular duties with the listed restrictions.
  • If the release date cannot be determined yet, the form asks the physician for an estimate. The current version dates from 2015.

Frequently asked questions

Who completes Part C of the Aflac Group continuing disability form?

Part C is the attending physician statement, completed by the physician assessing the patient's return-to-work capability. It covers diagnosis, treatment dates, complications preventing return, disability dates, the release date, prognosis, and restrictions and limitations related to the patient's occupation.

What if the return-to-work date is unknown?

Part C asks for the date the patient is released to return to work and, if that cannot be determined yet, an estimate. The physician also indicates when a fundamental or marked change is expected: within a month, one to three, three to six or six to twelve months, or never.
