Aflac Group Cancer Claim Form: Physician Statement

Aflac Group cancer insurance claims, underwritten by Continental American Insurance Company, use this seven-page form. The attending physician's statement on page 3 records the type of cancer, initial diagnosis date, treating physicians, hospitalization, surgery, and chemotherapy or radiation. For a first claim, the pathology report or exam with diagnosis is required.

Who completes it

The policyholder completes the policyholder and patient information and any transportation or lodging log, and signs with the patient. The attending physician completes the statement on page 3 and signs with degree and medical ID number.

Submitting and payment

How to submit it
The completed claim is emailed, faxed or mailed to Aflac Group's claims unit, and the employee can also file it through MyAflac. The signed HIPAA authorization (AGC06106) goes with it.

Good to know

  • Supporting documents include itemized bills (the UB-04 for hospital stays, the HCFA 1500 from the physician's office), chart notes, the operative report or surgeon's bill, and chemotherapy or radiation billing.
  • The physician states whether the patient received oral or topical chemotherapy; those claims need pharmaceutical statements.
  • Claims under the Lump Sum Cancer Plan also need a copy of the patient's birth certificate.
  • Benefits go to the policyholder unless written authorization assigns them to the provider; to assign, the policyholder sends a signed written request.

Frequently asked questions

Does the Aflac Group cancer claim need a pathology report?

Yes, for the first claim. The physician's statement asks whether the patient has been diagnosed with cancer and, if so, to submit the initial pathology report or exam with diagnosis. The policyholder section also asks for the date a pathologist first diagnosed the cancer.

Can Aflac Group cancer benefits be paid to the provider?

Only with an assignment. Benefits are payable to the policyholder unless Aflac receives written authorization from the policyholder or the healthcare provider to assign benefits to the provider. To assign, a signed written request goes in with the claim.
