Rehabilitation Training Deferment Request (Section 4)

Federal student loan borrowers enrolled in a vocational, drug abuse, mental health or alcohol abuse rehabilitation program can request a deferment, and the program certifies it. Section 4 of this nine-page Federal Student Aid form records the program's begin and end dates and the authorized official's certification that the borrower and program meet the conditions.

Who completes it

The borrower completes Section 2 (eligibility questions) and Section 3 and signs. An authorized official of the rehabilitation program, not necessarily a licensed clinician, completes Section 4 with institution name, OPEID if applicable, name and title, signature and date.

Submitting and payment

How to submit it
The borrower sends pages 1 to 4 (omitting pages 5 to 9) and any documentation to the loan servicer or holder; a separate request goes to each loan holder.
Who pays
Federal Student Aid sets no fee and the form does not say who pays for the certification.

Good to know

  • The program must be licensed, approved, certified or recognized by the Department of Veterans Affairs or a state agency for vocational rehabilitation, drug abuse, mental health or alcohol abuse treatment.
  • Services must follow a written, individualized plan with an expected end date, and the program must require a substantial commitment from the borrower.
  • The official should not complete Section 4 unless the borrower has completed Section 2 in its entirety.
  • Instead of Section 4, the borrower may attach separate documentation from an authorized official that includes all the requested information.

Frequently asked questions

Who completes Section 4 of the Rehabilitation Training Deferment Request?

An authorized official of the rehabilitation program, such as a treatment program director or clinician. The official enters the program's begin and end dates and certifies that the borrower and program meet all conditions indicated in the borrower's Section 2 answers.

What counts as a substantial commitment for this deferment?

The form defines it as a commitment of time and effort that would normally prevent a person from being employed 30 or more hours per week in a position expected to last at least 3 months. The borrower answers this in Section 2.

When does the rehabilitation deferment end?

On the earlier of the certified program end date or the date the borrower no longer qualifies. Perkins borrowers then receive a 6-month post-deferment grace period, and unpaid interest may capitalize on other loans when the deferment ends.
