USCIS Form N-648: Medical Certification for Disability

Naturalization applicants who cannot meet the English or civics test requirements because of a physical or developmental disability or mental impairment request an exception with Form N-648. A licensed medical professional examines the applicant and certifies the diagnosis, explains how the condition prevents them from meeting those requirements, and addresses whether the applicant can understand the oath.

Who completes it

A medical doctor, doctor of osteopathy or clinical psychologist licensed in the United States completes Parts 2 to 4 and the Part 7 certification. The applicant signs the Part 6 attestation and release of information; a guardian or surrogate may sign for an applicant who cannot.

Submitting and payment

How to submit it
Give the completed form to the applicant, who files it with Form N-400 or later. Every page must carry the same 09/25/24 edition date, and USCIS rejects an unsigned form.
Who pays
USCIS charges no filing fee for Form N-648; the medical professional may charge the applicant for the examination and for completing the form.

Good to know

  • The examination must be in person, or by real-time telehealth only where state law permits it.
  • The 5-page fillable PDF must stay together as one edition; USCIS checks that the edition date and page numbers appear on every page.
  • USCIS warns about fraudulent medical certifications and cites several criminal prosecutions tied to N-648 schemes.
  • USCIS has a Spanish information page about the N-648, but the form itself is published in English only.
