North Carolina MOST Form: Scope of Treatment Orders

This physician order sheet, recognized under N.C.G.S. 90-21.17, records the patient's choices on CPR, scope of medical intervention, antibiotics, and medically administered fluids and nutrition. It is honored across settings and by EMS. Clinicians complete it with patients or their representatives as part of advance care planning, and the original form travels with the patient.

Who completes it

A health care professional prepares it with the patient or representative, and a licensed physician, PA or nurse practitioner must sign and date it. The patient or representative signs too, or a signed copy is kept on file and noted in the signature field.

Submitting and payment

How to submit it
Nothing is filed; the original form goes with the patient whenever transferred or discharged, and the 03/2024 version must be printed on Astrobrights Pulsar Pink paper.

Good to know

  • The form's directions say use of the original form is required, so the pink original, not a copy, should be sent with the patient.
  • Document the basis for the orders and the mode of communication, such as in person or by telephone, in the progress notes.
  • MOST may suspend conflicting directions in an earlier health care power of attorney or living will; attach copies of those documents when available.
  • Review is recommended at admission, discharge or a substantial health change and required when preferences change. To void it, draw a line through Sections A to E and write VOID.

Medical Orders for Scope of Treatment (MOST)

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
