DC MOST: Medical Orders for Scope of Treatment

Seriously ill District patients can have their treatment choices written as medical orders that are valid in all care settings, including hospitals, until replaced by new orders. The MD, DO or APRN records CPR status, full, selective or comfort-focused treatment, medically assisted nutrition and antibiotic use after shared decision-making with the patient or representative. An electronic version, eMOST, is available through CRISP DC.

Who completes it

An MD, DO or APRN completes Sections A through C and signs Section D with a license number, and the patient or legal authorized representative also signs. A representative may sign only if the patient is mentally incapacitated.

Submitting and payment

How to submit it
Nothing is filed with DC Health; keep the original with the patient's medical records, or create or upload the orders as an eMOST in the CRISP DC health information exchange.

Good to know

  • Any incomplete section implies full treatment for that section, and verbal orders need a follow-up signature by an MD, DO or APRN.
  • Photocopies and faxes of signed MOST forms are legal and valid, and copies may be made for records.
  • eMOST, authorized by D.C. Law 23-62, lets providers create, upload and view advance care plans that care teams, including DC Fire and EMS, can see.
  • To void the form, draw a line through Medical Orders and write VOID in large letters; any change requires a new MOST.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a physician assistant sign a DC MOST?

The current DC MOST form calls for an MD, DO or APRN signature, and DC Health's MOST page names physicians (MD or DO) and APRNs only. Verbal orders are acceptable with a follow-up MD, DO or APRN signature in accordance with facility or community policy.

Is there an electronic DC MOST?

Yes. The eMOST is a cloud-based format accessed through CRISP DC, the District's designated health information exchange, using the MyDirectives for Clinicians platform. Users can also scan and upload existing paper MOST forms. D.C. Law 23-62 provides the authority for the electronic format.
