Wyoming Driver Vision Evaluation (WYDOT FSDI-914)

WYDOT Driver Services uses this evaluation when a driver needs a vision review for licensing. The vision specialist records acuity without lenses, with current lenses and with a bioptic telescope, whether the horizontal field meets Wyoming standards, and treatment for macular degeneration, cataracts or glaucoma. The specialist then marks one opinion on visual capability to drive and any restrictions.

Who completes it

A qualified vision specialist, an ophthalmologist or optometrist, completes and signs everything after the applicant information, including how long the applicant has been a patient and their medical or optometry license number.

Submitting and payment

How to submit it
Mail, fax or email the evaluation to WYDOT Driver Compliance, or upload it through oneWYO; incomplete forms may be returned to the specialist.

Good to know

  • Wyoming's license standard is 20/40 with both eyes and a combined horizontal field of at least 120 degrees, or 90 degrees in one eye if blind in the other.
  • If best acuity in the carrier lenses is worse than 20/100, the bioptic must correct it to at least 20/40; combined acuity of 20/50 to 20/100 may require a re-exam.
  • Checking a progressive ocular condition makes the annual Driver Vision Evaluation mandatory unless the specialist explains otherwise.
  • A road test recommendation is not allowed when Option 1, not visually capable of safe driving, is marked.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What are Wyoming's vision standards for a CDL?

The form lists 20/40 or better in each eye with or without corrective lenses and a horizontal field of at least 70 degrees in each eye for a commercial license. For a regular license it is 20/40 with both eyes and a combined field of at least 120 degrees.

How recent must the exam be for Wyoming's Driver Vision Evaluation?

If the evaluation is needed to meet an annual requirement, it must be from an exam within the last 3 months. A progressive ocular condition such as macular degeneration, cataracts or glaucoma triggers the annual requirement unless the specialist explains why it is not needed.
