Kansas DV-124V: Division of Vehicles Vision Form

Kansas's Division of Vehicles uses this one-page vision form when a driver is referred to its Medical/Vision Unit. The vision professional records distance acuity with and without correction, bioptic readings, horizontal fields, diagnosis and prognosis, then answers whether the driver needs adaptive equipment, a drive test, a medical exam or a Kansas Medical Advisory Board review, and recommends restrictions.

Who completes it

The patient, or a guardian if under 18, completes the general information and signs the release. An optometrist or ophthalmologist completes Section I, the vision report, and signs with specialty and exam date.

Submitting and payment

How to submit it

Good to know

  • The exam must be within the past 90 days, or 6 months after a seizure or consciousness occurrence.
  • Bioptic or telescopic readings are for the vision professional's use and aren't used to decide issuance or drive test requirements.
  • If the driver has a Cortical Visual Impairment diagnosis, additional testing is recommended; print and attach the CVI test results.
  • Choose up to 6 restrictions; corrective lenses only if the driver must use glasses or contacts, and write R beside a restriction to remove it.
