New Hampshire DSMV 649: School Bus Driver Physical

Applicants for a New Hampshire School Bus Certificate need a physical exam meant to detect deficiencies that affect their ability to drive a school bus safely. On DSMV 649 a licensed physician records vision, hearing and reflexes, notes any illness, surgery or prescription medication from the past five years, and certifies that the applicant is physically capable of operating a school bus.

Who completes it

A licensed physician completes the whole exam form and signs the Licensed Physician's Certificate, adding title, license or certificate number, state of licensure and its expiration date.

Submitting and payment

How to submit it
The completed form goes to the New Hampshire DMV Pupil Transportation Section, which oversees school bus driver certification.

Good to know

  • Vision is recorded for each eye with and without glasses, along with a color test, horizontal field and any evidence of eye disease or injury; hearing is recorded for each ear.
  • Reflex items include Romberg, pupillary light and accommodation responses for each eye, and knee jerks on both sides, with space for comments.
  • The history checklist names diabetes, seizures or fainting, vertigo, stroke, head or spinal injury, kidney, cardiovascular and psychiatric disorders, among others, with room to explain any that apply.
  • The School Bus Certificate is separate from the driver license, and DSMV 500 checklists set out the driver qualification file. The current edition is DSMV 649 (Rev. 6/24).

Frequently asked questions

Who can complete New Hampshire's school bus driver physical?

A licensed physician. The certificate asks for the physician's title, license or certificate number, state of licensure and expiration date. The physician certifies familiarity with the applicant's current condition and that the applicant is physically capable of operating a school bus, knowing the DMV will use it to decide eligibility.

What history does the New Hampshire school bus physical ask about?

Any illness, surgery or prescription medication in the past five years, with listed conditions such as diabetes, tuberculosis, rheumatic fever, ulcer, seizures or epilepsy, vertigo, stroke, head or spinal injury, kidney or cardiovascular disease, asthma, muscular disease and psychiatric disorders, plus a written explanation.

Is DSMV 649 the same as a CDL medical card?

No. DSMV 649 supports the state School Bus Certificate, which is separate from the driver license. The DMV separately requires a valid medical card for anyone driving a vehicle over 10,000 pounds GVWR or designed to carry 16 or more passengers including the driver.
