New Hampshire DSMV 603: Tinted Window Medical Waiver

New Hampshire bars aftermarket tint on the windshield, but a physician's certification on DSMV 603 lets the DMV issue a permit for tint on the windshield and the windows beside the driver, down to 35 percent light transmittance. The physician names and describes the condition, explains how tinting alleviates it and certifies medical necessity under RSA 266:58-a and Saf-C 2500.

Who completes it

A physician licensed to practice medicine in any US state completes and signs the lower section. The applicant, or a parent or guardian for a minor, fills in the personal and vehicle details, and each vehicle's registered owner signs.

Submitting and payment

How to submit it
The applicant mails or faxes the signed application, dated within six months, to the DMV Director's Office with photocopies of the registrations for up to two vehicles; if approved, the permit is mailed to the applicant.
Who pays
The DMV charges no fee for the waiver; the form does not address payment for the physician's certification.

Good to know

  • All three certifications must be checked: sunglasses or other protective equipment will not help, tint is recommended for the windshield and front side windows, and the applicant is under the physician's care.
  • An approved waiver lasts two years. Checking the box that the condition is likely to last at least four years allows a four-year waiver instead.
  • Renewal takes a new application signed by the physician within six months of submission, sent with photocopies of the vehicle registrations.
  • The permit covers only the vehicles listed on the form, up to two. The current edition is DSMV 603 (Rev. 9/2025).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can an out-of-state doctor sign New Hampshire's tinted window medical waiver?

Yes. DSMV 603 must be completed and signed by a physician licensed to practice medicine in any US state. The physician also certifies that the applicant is under their treatment and care and that sunglasses or other protective equipment will not alleviate the medical condition.

How long does a New Hampshire tint waiver last?

Two years once approved. If the physician checks the box stating the condition is likely to last at least four years, the waiver runs four years instead. To renew, the applicant submits a new application signed by the physician within the previous six months, with registration copies.

How dark can windows be with a New Hampshire medical tint waiver?

With a waiver, aftermarket tint may be applied to the windshield and the windows to the left and right of the driver at not less than 35 percent light transmittance. Without one, the front side windows must allow at least 70 percent and windshield tint is not allowed.
